Step 1: Receiving DBS Sample Collection Kits

You will receive the kits with the following content per kit:
• 1 collection form (DBS).
• 1 Test request Form (only for remaining kits distribuited before April 2024).
• 2 patient informed consent forms.
• 1 plastic envelope for return.
• 1 silica gel bag.
• 1 external envelope for shipment.
• 1 informative flyer of the program.
• 1 Physician Statement (only for remaining kits distribuited before April 2024.)You will receive the kits with the following content per kit:

Step 2: Requesting Shipment for DBS Sample Collection

1. Log in to the RDGenePP App and register if it is your first time.
2. Select New Test Request Form and complete all fields.
3. If Local laboratory collects sample from your patient, this TRF will be sent automatically to them for coordination directly with patient.
4. If you are to collect sample from the patient please collect the sample and after completing TRF, select new pickup request so local laboratory can pickup samples at your office or hospital.

Step 3: Requesting Shipment for DBS Sample Collection
Once samples are in transit to DLE Or Revvity, you will receive a notification and a link to the site to view your results: RDGenePP App.
Step 4: Results available
A notification will be sent to your email once the results are available
on the RDGenePP App.
